Common causes are:
- Impaired Mechanics: Weakness and muscle imbalances can lead to a limited range of motion, while joint stiffness and muscle inflexibility can affect posture.
- Injury can result from a sudden load or impact on the tissue.
- Chronic compensatory mechanisms, such as avoiding certain muscle groups and overusing other tissues due to previous injuries or fear of future injuries, can also contribute.

- Maintain proper neck and thoracic (mid-back) posture.
- Decrease tension in the upper neck muscles.
- Enhance flexibility of the chest and neck muscles.
- Utilize manipulation, manual joint mobilization, and manual traction to improve joint mobility.
- Strengthen the muscles responsible for flexing the neck and shoulder blades.
- Use massage, instrument-assisted mobilization, and cupping to increase tissue mobility.
- Incorporate specific nerve exercises to reduce nerve sensitivity, which can cause numbness, tingling, and pain that extends down the arm.
- Implement functional and plyometric exercises, such as throwing, catching, pushing, pulling, and pressing.
Here are some important facts to keep in mind:
- Tissues can recover on their own, but sometimes they may become stuck in the inflammatory stage of tissue repair and need assistance to progress toward recovery.
- MRI and X-ray imaging may not always accurately reflect the source of pain. Imaging can reveal tissue damage despite a lack of pain or functional impairment, while pain may exist despite no significant findings on imaging, typically due to excessive nervous tissue sensitivity.
- Soreness after exercise is not always a cause for concern, as true strength gains require micro damage that leads to tissue growth. The soreness should be used to determine if the tissue is ready for more load, and training intensity should be reduced if it is not.
- With proper instruction, mechanics, and progressive tissue loading, it is almost always possible to return to previous or desired activities.
